Shepherds Promise Care Service Pty Ltd is authorised to provide community nursing services to eligible DVA clients. Care is based on assessed clinical needs and a valid written referral.

Medication support
Continence and catheter care
Nursing reviews and clinical monitoring
Nursing support for chronic conditions, recovery after hospital care, and palliative care
Personal care
Assistance with showering and hygiene, dressing and grooming, toileting, mobility and transfers, and eating where clinically required.
Care coordination
We work with your GP, hospital and other healthcare professionals to coordinate care that meets your assessed needs.
Eligibility:Eligibility depends on your Veteran Card and assessed clinical needs. For a Veteran Card – Specific Conditions, we confirm eligibility with DVA before services begin.
Referrals
A valid written referral is required from a GP, treating hospital doctor, hospital discharge planner or nurse practitioner specialising in community nursing.Contact our team for instructions on submitting your referral.
